Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.7) Gecko/20040608 
Firefox/0.8.0+

downloaded from:

I chose custom installation, and only quick-launch toolbar icons.
The icon in the toolbar was the document icon.

Right-click -> properties -> change icon had the application icon as well.

This is a fairly important branding issue we should get worked out.
Just to clarify, by "quick launch" I assume you mean the Windows quick launch
toolbar, not the Seamonkey hack.

yes, I mean windows quick launch toolbar.
Can reproduce with:
Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.7) Gecko/20040626 Firefox/0.9.1

on 'one' of my machines. The other machine works fine. Not sure what the
difference is. Tried deleting mozilla/firefox related entries from registry
without any luck.

This is related to bug 171349, if not a dupe.  I'm not sure, though.
It will be a dup. Mark it as such. The icon is sourced from the same place - the
app's exe file, so all such bugs are the same.
