The admin I had contact with replied back to me.

He said he wholeheartedly agrees with:

So, he disabled the config.xml now.

He added:
The item "MANUELL_BEARBEITEN: IHRE_MEDMA_ID@medma.ad.uni-heidelberg.de" was a workaround, but not a solution, because many users do not exactly understand, was they are supposed to do at this point. First, only one language can be used, and second, many users do not understand YOUR_MEDMA_ID. :-( In the log, I can see that some users need 2 or 3 attempts to get the correct setting. Many users have no technical affinity and are completely overwhelmed and contact IT support. Mail out of the box is something else.
