It looks like this is unrelated to any of my changes in that pushlog at least, I've tested backing out those three patches locally and can still reproduce the issue. I've created a test case which I've attached to here. To reproduce, set "media.wmf.vp9.enabled" to false, then open the video and the access violation should occur.
