**Steps to reproduce**
1. Reach the payment form on https://petsasa.co.ke
2. Fill in manually all the CC info and submit the form

**Expected result**
* All CC information filled in the payment form should be captured in the Credit Card save doorhanger and saved on credentials management page

**Actual result**
* Cardholder name is not captured in the CC doorhanger or stored on credentials management page with the CC expiration date

* On Chrome the save CC doorhanger isn't displayed.
