If there are any non-default permissions I'd expect the permissions icon to show up to the right of the lock icon, and list all such permissions, making them easy to revoke. If the page requested permissions that the user rejected, the icon for the relevant permission normally remains visible on the page. When do users need the page info dialog to manage permissions, in your experience?
