Description:
Once Fixed Dialing Numbers is enabled and the SIM PIN2 is correctly entered, there is no visible icon UI on the status bar that appears.

Repro Steps:
1) Update a Flame to BuildID: 20140530040207
2) From the homescreen, tap on the "Settings" icon.
3) Tap on "Call Settings"
4) Tap on the SIM you're using (if device is multi-SIM capable)
5) Tap on Fixed Dialing Numbers
6) Tap on the toggle switch next to "Fixed Dialing Numbers" to enable.
7) Type in correct SIM PIN2.
8) Tap "Done" in the top right corner and observe that there is no visible icon in the Status Bar.

Actual:
No visible status bar icon indicating FDN is enabled.

Expected:
A visible status bar icon appears indicating FDN is enabled.

This issue reproduces in Flame 2.0, Buri 2.0, Buri 1.4, and Buri 1.3. There is no visible status bar icon in Flame 2.0, Buri 2.0, Buri 1.4, and Buri 1.3.

This bug can be repro on latest build of Flame KK v2.2&2.5&master 512mb and Aries KK v2.5&master by  the STR in Comment 0.

Actual results: No visible status bar icon indicating FDN is enabled.
