Unfortunately, the given information is not enough to analysis this problem.

I want to know that it is reproduced after reinstalling - remove and then reinstall application.
In addtion, could you provide DB file or log for the futher investigation?

Homescreen app gets the data of icon from DB using XML HTTP Request after inserting it to DB just once on the first installing.

Possible cases :
 - Icon of applications is invalid.
 - DB has wrong icon data
 - XHR request is fail or gets the unexpected response.

Mass update: Resolve wontfix all issues with legacy homescreens.

As of 2.6 we have a new homescreen and having these issues open is confusing. All issues will block bug 1231115 so we can use that to re-visit any of these if needed.
