Ran into this on Windows Vista 32-bit, using Firefox 37 beta 7 (build ID: 20150319212106).
I don't have proper STR, but it happened right after a flash install, when I've restarted the browser. Tried several other times to reproduce it, without any luck.
Seems like only the 32-bit versions of Windows 7 and Windows Vista are affected.
