Password Manager onDOMFormHasPassword should mark all username fields it finds as usernames.

When the autocomplete menu pops up, it should provide both the username list and the formHistory deduped.
The key icons in bug 1189618 would help differentiate login and form history results. I'm not sure where the footer should go though?
