satdav, it's extremely unclear what you're asking for here.

we need to know:
- exactly what data you want to collect
- where to create a bug (product/component)
- how the fields collected relate to bug creation

bug 1140215 is a nice example of the level of detail we need.


that said plugin-check::database is a very low traffic component, and there doesn't appear to be may bugs filed that require further information from the reporter.  can you also provide a reason why this form is required.

glob, we don't need this form, this component is very low traffic, it's mostly employees filing bugs and fixing them immediately and we don't need to waste time creating a form.
