Opening up the organizer and looking at 1.0 Release crash report signatures shows a number of crashes in libAVFAudio.dylib

Code looks good but I was unable to replicate the issue as well. When I tried running the app in the simulator and removed my headphones the notification was not fired so I'm not sure if I'm doing it right.
