On large sites, like BMO, custom fields can out-number non-custom fields.

Searching for fields where custom = 0 is common, and that field is not indexed. Indexing that field would provide a slight performance improvement and I fail to see the downsides.
Wouldn't custom + obsolete make more sense than custom alone? You often want active custom fields.

looks like that is an even better win.
Summary: fielddefs.custom should probably be indexed → fielddefs custom and obsolete should probably be indexed
