Pre-requisite: 
Go to about:addons > Plugins 
Ensure Shockwave plugin option is set to "Ask to activate"

STR:
1. Go to Google.com
2. Observe Flash plugin icon next to location bar

Expected: There is no Flash plugin icon (as in released FX 49.0.2)

Actual: Adobe flash icon next to google.com.

Note: This only happens when Shockwave plugin option is set to "Ask to activate". Tested with fresh profile.

Kanchan, please find a nightly regression range. I suspect this is bug 1308677, caused by bug 1292317, but the symptoms are different enough that I'd like you to check.

Looks like the following bug has the changes which introduced the regression:

Please not build id from 20161003134958 to 20161006195807 is not showing shockwave plugin in about:addon >plugin. I marked these as bad too. 

My reported issue started from build ID 20161006195901
