I've seen this happen a couple of times now:

I think it might be due to when someone reviews by looking at the PR diff in bugzilla?
This has happened a few times since, the most recent in bug 1390169 comment 12.
This happens half a dozen times a month.
Happened again in bug 1445295 (has happened other times since comment 2; just haven't commented for each).
Israel: Can you see about reproducing this in the dev vm?
