1483412 - Make the telephone number field required on the payer form if requestPayerPhone is true

The asterisk should be shown and the save button should be disabled when it's missing. This may get fixed by bug 1481481.

The reason this field is special is because the telephone number isn't required on the billing or shipping address form AFAIK.

Verified - fixed on the latest Nightly 65.0a1 (2018-11-02) (64-bit) on Windows 7/10, Mac OS 10.13. and Ubuntu 16.04.

The phone number is required only on the Payer Contact Forms. 
On the shipping/billing address forms, the phone number is not required - as expected.
