When I press ctrl-shift-t to open the url box, I get two error boxes. The first
is titled ASD_TP02.exe - Application Error. The box says: "The application
failed to initialize properly (0xc0000005). Click on OK to terminate the
application." The second box is identical to the first.

Actual Results:  
I get two error boxes. The first is titled ASD_TP02.exe - Application Error. The
box says: "The application failed to initialize properly (0xc0000005). Click on
OK to terminate the application." The second box is identical to the first.

Expected Results:  
It should have opened a box for me to type in a URL

Ctrl+Shift+T is not supposed to do anything in Mozilla.

Hmmm.. what happens if you press Ctrl+Shift+T when Mozilla isn't running?

I suspect that it's trying to launch an application on your system called
ASD_TP02.exe, and that this has nothing to do with Mozilla.

You should do a file search on your system to see where ASD_TP02.exe is. It's
not a Mozilla file. Perhaps you have a shortcut pointing to it from your
desktop, start menu or somewhere else. If you click on properties you might see
"shortcut key" ... Ctrl+Shift+T.

Bug shown to be intrinsic to bug reporter's system, not to Mozilla.
