232181 - either the form manager isn't getting the form info to save, not saving it or not finding any to show

Every time the form manager asks if I want to save the filled in info I click on
Yes (except for twice, see below). If I then go to Tools==>Form
Manager===>Manage Sites, there's nothing there: The Forms Never previewed window
is empty & the Forms Never Saved window contains two URLs: one for my router
(192.168.1.1), & one for a page in which I wasn't interested:
www.creaturesinmyhead.com/cremail-send.php. All the info from the many forms
whose info I wanted to save are nowhere to be found.I also had this problem with
the previous OS/2 version: IBM Web Browser for OS/2

Reproducible: Always
Steps to Reproduce:
1. Go to a page containing as form
2. Fill in the form
3. When asked if the info should be saved, click Yes.
4. Go to Tools===>Form Manager==>Manage Sites
5. notice that there's nothing there. 
Actual Results:  
I only looked there after having filled in many forms, & there was no site
listed in the Forms Never Previewed window, except for the two in the Forms
Never Saved window.

Expected Results:  
If I've understood the help about forms correctly, the sites from all of the
forms that I filled in should have been displayed.

I've never changed a theme, so I assume it's the default & when I go to
themes.mozdev.org/ it shows "You appear to be using: Works with Mozilla 1.3." &
I don't think my hardware configuration is all that unusual.

You're using an old version of Mozilla. Please try to reproduce this with the
latest one, v1.6.

This is a VERY old Mozilla.

Please at least check with 1.4.1
