Use a proxy which authenticate using a NT-DOMAIN/NT-USER and PASSWD to logon;
the dialog asking these informations popups several times when loading a page,
usually once for each frame, iframe, image (it seems once for each GET of the
browser...). Sorry but can't give detailed information about the proxy. The
problem occurs even if account informations are stored (saved) by the browser.
The problem doesn't occur with 0.9.3 version.

2. Visit a web site

The proxy login dialog popups several times asking for login and password

Account information be asked just once... :-)

It's not resolved. It's still there. Furthermore, this bug is UNIX-specific, Windows version doesn't have that.
