Auto-complete creates duplicate entries in several fields:
1) login (and associtated password)
2) Name
3) e-mail
4) address (usually only Address Line 1 if form supports 2 lines)
5) telephone


Reproducible: Sometimes

Steps to Reproduce:
1. fill out a form
2. fill out the form again
3. fill out the form again

Actual Results:  
Firefox collects and stores two entries in auto-complete

Expected Results:  
One entry would suffice


it varies from site to site, and is hard to duplicate with a fresh profile since
you usually need to re-visit a site.
Sometimes, it carries over login/password from another site (assuming you use
the same password).
It doesn't seem to matter which auto-complete entry you select to auto-fill
passwords (but if you have the same login with a different password, it goes
with the most recently typed password--non-duplicates don't suffer from this).
It only seems to collect two of any field.
Sometimes, even though it auto-filled the password, it asks if you want to save
the password.

This bug has had no comments for a long time. Statistically, we have found that
bug reports that have not been confirmed by a second user after three months are
highly unlikely to be the source of a fix to the code.

this problem in the latest version of the product (see below for how to obtain a
copy) or, for feature requests, if it's not present in the latest version and
you still believe we should implement it, please visit the URL of this bug

If it is not a problem any longer, you need take no action. If this bug is not
changed in any way in the next two weeks, it will be automatically resolved.

I don't know if this is exactly the same issue, but whenever I have to fill in my email address in a form, Firefox presents me with about a dozen identical lines of my email address.  One would suffice.
I am also experiencing similar behaviour with Version 24.0 (also happened with a few previous versions).

The duplicate auto-complete entries never seem to contain more than 2 of any given entry but will contain doubles of whatever values are entered more than once.
