Details

Rendering a page with — or “ entities does not render
anything at their locations.  If the font is missing those glyphs
(as I think ISO-8859-1 fonts do) it should fall back on -- or ",
or else show that the entity is not supported by rendering the
text "—" etc. (Netscape 4.7 does the latter--that's ugly if
you ask me).  Falling back to -- and " is clearly the right thing.

ldquo was already in the transliteration table. mdash was a single hyphen '-',
and is now a double hyphen "--". Marking FIXED.
