This continues to be an issue for me, doesnt matter what website I'm visiting, my browser will shut down out of the blue with the message to send report to MS, I have installed talk back and everytime I attempt to submit the error I get an error from talk back that my message wasnt set because of proxy error.

Reproducible: Always
Component: Error Console → General
QA Contact: javascript.console → general
This is most likely an extension problem. Please run Firefox in its safe-mode to find this out, or disable them in the extension manager. You can also try a new profile.

For follow-up questions see the mozillazine forums:

Additionally open Talkback and switch on the proxy settings inside the options. If you are behind a proxy Talkback can't send anything without configuring this part. Let us know the incident id.
