410143 - Clear icon cache more aggressively

From bug 388649 comment 18 we clearly aren't doing enough to clear the icon cache for users who run a lot and quit infrequently, so we need to do more.

We're probably going to need to start a thread (after we've been running for a while, to give the bookmark favicons time to load) that slowly eats old entries while running.

Unfortunately, I lost my huge site icon cache the other day :(  (It was still about 3K files dating back to 2006 and 20 MB or so, though.)

3.5K files, dating back to December 2009, 23.3 MB.  I can zip it up for safe-keeping, but I don't know if it will be useable if I need to restore it (I seemed to think it was tied to the profile state or something; I can't remember what caused me to lose the last one before).
