If you look at the main toolbar with the back/forth keyhole, stop, reload, etc., you'll notice that there's more padding on the top of the toolbar. It's unnoticeable for most of the items on the toolbar, but for the keyhole it looks really unbalanced, since the back button is so close to the edges. The entire toolbar (excluding the borders) should be moved 1 px higher so everything looks balanced.

For a native appearance, we currently mimic Windows Explorer where toolbar icons aren't perfectly centered, either. As for the keyhole, see bug 415329.
