Sometimes, the download manager displays only a blank instead of the filename. This happened to me only using FlashGot (not regular save link as), so it's probably a bug in FlashGot too, but I think that the download manager should always display the filename, regardless of the way it was added.

Reproducible: Always

Steps to Reproduce:
1. Install FlashGot
2. Go to http://www.konjak.org/artwork.htm
3. Download the next-to-last image, tango_stand.jpg using the "FlashGot link" context menu item (also happens with FlashGot selection)
4. Wait for the file to finish
Actual Results:  
The filename displayed in the download manager is blank

Expected Results:  
The filename should be tango_stand.jpg

The screenshot shows the bug in action - the blank entries in the download manager are the tango_stand.jpg file.

Not sure, it happens only ocassionally, and I need to work, so I can't run FF in safe mode for extended periods of time.

But it did happen once more - this time, the filesize and website are missing (not sure if it's the same bug or not), and I'm 100% positive that I used the normal download manager (not FlashGot). The screenshot shows that when I tried to download the identical file again, both filesize and website are shown correctly (so I wasn't able to reproduce it).

I've found it.
FlashGot passes its link.description property as the displayName parameter for the download.
Sometimes this description (which is usually the content of the link, or the alt attribute for images) is empty, like in your case.
I'm "fixing" this in FG 1.0.3 (to be released tomorrow) by using the destination file leafName, instead, so mimicking the default download manager behavior.
