When using the shortcut ctrl + T tabs are not open, only works if I is on file>new tab

Works for me, are you sure you are testing on a clean page? (No plug-ins)

Very likely bug 78414. This happens when Flash, Adobe Reader, or other plugin content has focus.
Agree, WFM on Mozilla/5.0 (Windows; U; Windows NT 6.0; en-US; rv:1.9.1b1pre) Gecko/20080905031348 Minefield/3.1b1pre. Make sure you have tried in Safe Mode, and that you are not on a Flash, Adobe, etc. page. Try reproducing this bug on a website such as www.mozilla.org.
