The menu bar throbber should be automatically shown when "Always show the tab bar" is unchecked, or perhaps only when the tab bar isn't shown.

To me, it doesn's happens always, it only happens a few times but very usually. Well, i do not understand english so nice to know everything written in the original description, but i think that it is the same problem that i have: Sometimes, when i open another window or i go to the desktop, the Firefox "icon" doesn't shows at the menu bar. Thank you!

I found an easy fix:

Go to "View" --> "Toolbars" --> "Customize..."

You should see an "Activity Indicator". Drag it to where you want it to be (typically the upper right corner, right below the Windows "X" button).
iworms: This bug is about that being done automatically.
On the latest nightly this is still an issue. As a user who frequently only has on tab open I disable "always show the tab bar" as it eats up screen real estate. Thus I have no idea if a page is loading or not without some sort of throbber or other indication.

Have you tried the suggestion in comment 4? Also, there is a bug filed to make that toolbar throbber match the tab throbber (bug 603659). Also, have you seen bug 603594? I think that may be a dupe of this bug...
