4958 - Some pages will not paint if previously visited, or back button used

Using Mozilla-Win32 (4-10-99) nightly build.

1. Load a frameset page into Apprunner (absolutely verifiable with above URL).

2. Close Apprunner.

3. Open Apprunner and try to view URL #1.

4. Apprunner reports "URL successfully loaded", however page never paints.

Deleting the cache folder clears this up.

It seems to be related to history somehow. Another way to see this bug happen on
non-frameset pages is to visit http://www.colostate.edu and click on the
"Prospective Students" link in the green menu bar on the left. Then push the
back button. When the original page fails to load you will no longer be able to
look at http://www.colostate.edu until you delete the cache folder.

I already have enough blockers and crashers for M6.  If I fix them in time for
M6, I'll move some of these bugs back to the M6 milestone.

I'm keeping the M7 milestone for fixing XML bugs.
