Using a second monitor (vertical orientation) when Firefox starts the proxy authentication dialog sits top of screen half way across both monitors.  After successful login the dialog remains (and is blank) and cannot be removed.  Only work around I've found is to quit and re-open FF until it goes away.

Steps to Reproduce:
1. open Firefox
2. login at proxy authentication dialog
3.
Actual Results:  
 After successful login the dialog remains (and is blank) and cannot be removed.

Expected Results:  
Dialog would close

No reply, INCOMPLETE. Please retest with Firefox 3.6.13 or later and a new profile (http://support.mozilla.com/kb/Managing+profiles). If you continue to see this issue with the newest firefox and a new profile, then please comment on this bug.
