Word on the menu should be changed after user submit the survey:
Change "Survey for New Test Pilots" to:
option 1: gray it out, so people understand they cannot do it again
option 2: change wording to "Thanks for the survey" or things like that after they submit it.

Reproducible: Always
Done in 0.1.2 (although this depends on SurveyMonkey showing the 'thank you' page, so if the user e.g. clears cookies then it will go back to being available (although not blinking 'new').)
