To start the process you need to fill out the Committer's Agreement - http://www.mozilla.org/hacking/committer/committers-agreement.pdf and attach an SSH key to the bug. Make sure to reference the bug number on the form when you send it to Erica. https://wiki.mozilla.org/Commit_Policy:Current_Procedures#CVS_.28cvs.mozilla.org.29 outlines the vouchers, etc needed for access.

I've already done the committer's agreement, ssh key, vouchers, etc to get hg access in bug 514995. What do I need to do just to add the ability to commit to the 1.9.0 branch CVS?

tn already has Hg access, as per bug 514995. As such, just carry this over.
