578900 - Playback can get stuck mashing pause/play near end of media

If you play a short ogg file, such as bug461281.ogg from the test suite, and mash the pause/play button while playing, sometimes the playback head will stop about half way through, even though the entire file plays through. The audio is finishing playing, but perhaps due to the frequent pausing and unpausing, we're not updating the current playback position (or properly signalling end of playback) after the sound has finished playing. Additionally, the controls are jammed, hitting pause/play does nothing.

STR:
1. Load content/media/test/bug461281.ogg in a video document.
2. When the current playback position reaches about half way through the media, mash the pause/play button.
3. Observe that sometimes the controls get jammed; pressing play does not cause the current playback position to advance, i.e. we're stuck and can't play or restart playback.
