In Firefox 3.6.x when you right click, Open in New Tab is the second option down.

In Firefox 4.0 when you right click, Open in New Tab is the first option down.

So it is irritating for users accustomed to opening in new tab when they instead open in new window.

Solution: Remove Open in New Window completely, and give user the option to show it.

Rather than cause unnecessary frustration for users upgrading Firefox 4.0 it should be removed by default, as explained here: http://www.makeuseof.com/tag/how-to-remove-the-open-in-new-window-option-in-firefox/

I did a clean install of Firefox 4.0 and now the above instructions no longer work. More specifically there is no Chrome folder?

Also why can't Open in New Tab be the second option down, when right click, as in Firefox 3.6?

where is it already covered?

RE switching window and tab this is intentional and has been fixed by bug 322736.
