this is clearly caused by either a wrong change in userChrome.css or an add-on. Try safe-mode http://support.mozilla.com/kb/Safe+Mode you can also disable add-ons one by one to figure out who is the culprit.

Resolution: --- → INVALID
Culprit was PageRank Client 1.0 add-on when the Google Pagerank tool was dragged to the right of the address bar.
