Details

It sends an email for the comment change and an email for the review response. Both of these emails contain basically the same information. It would be better if bugzilla just sent one.
This is not really a Splinter or BMO specific issue as this is part of the upstream design. When you comment on an attachment through Splinter or the standard attachment details page, if a flag change happens as well, Bugzilla will send an email with the comment change, flag change as a normal notification. And then it also sends an email outlining the flag change as well to the requestee if one and any other cc people the Bugzilla admin has configured for the flag. This way a requestee can be someone not even related to the bug and will get the flag request which stands out more such as when a attachment needs review, etc.

This has been this way for a while and this request might be a duplicate so I am changing to upstream product to see what others think.

I'm sorry I still don't understand. What's the value of sending the the comment change notification to the requestee?

If the requestee for whatever reason is not associated with the bug, they will see the comment that is being added related to the flag change itself. In the context of a review they will see why they review was granted or denied. So the comment is important information. To see any other changes not specifically related to flags, they will have to be on the bugs cc list or one of the other roles.
Paraphrasing Frédéric Buclin: the motivation for two mails is for when recipient has separate workflows for flagmail and bugmail wants the email to end up in both. (e.g one for action and one for archiving)

This should be more configurable when bug 97761 is fixed.
