Unclear test case status between written and draft

Details

After a user creates a test case it is in a blank status. After being approved once it goes into Draft status. It's unclear what the difference is between the initial state and the Draft status- shouldn't it go directly into draft status after being written?

Rebecca and I were talking:  The Check/X buttons for not-approved and Draft/Rejected are confusing.  Also the "Locked" status is misleading.

Locked status should become "Disabled"
Instead of showing the Check/X buttons, just show draft.  Let's remove the "approval" concept completely.  Automatically approve everything into draft status when it is created.
