Opening this bug to collect suggested improvements to the encoding tables. 
If you have any such suggestions, this is the place to put them. Let's not
waste glyphs or combination of glyphs by not encoding them.

amalg displays as ? in my Mozilla.

Looking at the encoding pages, slot 0x71 in CMSY should be amalg=0x2A3F not 
coprod=0x2210.  Slots 0x60 and 0x61 in CMEX are used for coprod.

There seems to be a left bracket symbol missing in the following rendering example,

It appears a left bracket is missing. Look at the row titled "B0", 8th places to the right, in between the parenthesis and the left bracket. 

In TeX speak, this is the "/biggl" cmex10 symbol. Is this due to the non-standard encoding? I haven't been able to reproduce this bug in mozilla (mostly because I'm new to MathML).

I'll close this bug.  It's mostly obsolete as we now don't do encoding conversion for regular characters.  We still have tables for stretchy characters but I'd prefer to have separate bugs for any changes there, rather than accumulating on a single bug.
