When you are using the Add-on Builder to preview a restartless extension which is hosted on AMO, the installed extension is not visible in the add-ons manager. Also trying to uninstall it will leave it running in Firefox. See the following example:

1. Goto https://builder.addons.mozilla.org/addon/1023834/latest/
2. Hit the preview eye icon
3. Install the Add-on Builder if it hasn't been installed yet
4. Wait until the extension has been installed
5. Check the Add-ons Manager
6. Hit the preview eye icon again
7. The extension is still running even the Builder claims the extension has been uninstalled

I'm not sure what's going on here and into which component it really belongs. Dave, I'm sure you can better analyze it.
My understanding is that the add-on builder helper uses a hacky method for previewing add-ons, I think we have a bug open to fix that, daniel?

Isn't the rewrite (bug 712041 or bug 714815) of Helper going to tie into the Addon Manager more correctly?

Yes, installing add-ons (hopefully we can install many at once!) correctly so that they are represented in the Add-ons Manager is a P1 for the refresh ;)
The builder project has been discontinued as of April 1st, 2014.
