Need fields added to these forms to allow Reps form to indicate if this is an FSA-specific event so we can better track gear and budget distribution:

1. First Field prompt: "Is this event being hosted by a Firefox Student Ambassador(s)?" and the individual filling out the form be able to either click 'Yes' or 'No' 

2. Second Field prompt: "Is the main audience of this event Firefox Student Ambassadors?" and the individual filling out the form be able to either click 'Yes' or 'No'
