Steps to reproduce:
On some sites I get all available icons within the address-bar. e.g. Gmail.
Maybe you have to refresh the site several times to get this issue.

Uh oh, that doesn't look good. Can you reproduce this when you restart with add-ons disabled? (there's an option for this in the main menu's "help" menu)
I've seen several add-ons that cause this, so I suspect an add-on issue.
