In our meeting on April 15 we discussed additional data that Legal requires as part of the registration process.  Please review and either approve We need page 5 of this document:  https://drive.google.com/a/mozilla.com/file/d/0B5fr2uXG8zVqRW4zZTZ6NGE1dFE/edit?usp=sharing OR provide corrections in this bug.

This bug re-iterates email request for same sent on May 9.  Sign-off was requested for today, but can we get this by noon Friday?  Developers are now starting this work.

The list of fields is good as it is but I believe you are missing:

1) "Company Address" after "Mobile" but before "City" 
2) "Company Zip Code" after "State" but before "Country"

3) "Company Website" (seems to me this is missing and is a nice to have, as we might need it to research something (like corporate name/location etc.) and could be different than the email domain)
