In Firefox OS nightly, happens when you toggle a `disabled` class with `opacity: 0.6` on a container a few times.
I'll try making a test case of this with better STR.

These screenshots happened in bug 1033392. I had to work around this by targeting "li.disabled > {a,p,h2,label,select}" instead of "li.disabled".

Wonder if it's the same underlying issues as bug 1050468.
