Running today's Nightly on my laptop, I was watching a Youtube video via Flash when suddenly the browser stopped painting. The process wasn't hung, so I attached a debugger and noticed a flurry of C++ exceptions being thrown from inside d3d11.
This was all happening at http://dxr.mozilla.org/mozilla-central/source/gfx/layers/d3d11/CompositorD3D11.cpp#642. The exceptions were being caught inside D3D11 and subsequently CreateShaderResourceView was returning DXGI_ERROR_DEVICE_REMOVED.
Note that I hadn't actually done anything to my graphics device. Maybe Flash messed something up?

A lot has changed since this report, but here are the crashes that are reporting the same message: https://crash-stats.mozilla.com/search/?product=Firefox&graphics_critical_error=TextureSourceD3D11%3AGetShaderResourceView+CreateSRV+failure&_facets=signature&_columns=date&_columns=signature&_columns=product&_columns=version&_columns=build_id&_columns=platform#facet-signature
Not a lot of them overall.
