There were a few rough spots with the first iteration of the review history, including canceled flags. With bug 1067808 resolved, we will show "canceled" statuses in both those cases. Currently we do not report on feedback flags, so "changing" a review? to feedback? is the same as canceling the review? flag. 

Attached is a picture of the current (top) review history and the new (bottom) one that I suspect will make it out in the next BMO push.

(Note the data in the screenshot is dated, as I don't have access to anything more recent than May, but I'm confident this issue is resolved.)
