1089422 - Fix dropdown showing up after state/province has been selected on static donation form

After I select the 'US' as my country, when I use the  'State/Province' drop down it shows all of the states/provinces in the  whole wide world. Is there any way that we could make this only US  states once that's identified as the country?

As discussed in IRC, using css on optgroups only works in some browsers, so Scott's gonna try a javascript approach.

This is great. Follow-up ask: can it default to "none" for all other countries?
