Steps to reproduce: 1. Load MP-stage “My Submissions” page (https://marketplace.allizom.org/developers/submissions) 2. Click the “Message” link for a published app. 3. Click any “# notes” link. 4. Click the “Reply” button. 5. Click the “Attach another file” link. Expected results: Another line containing “Attachment” and “File Description” fields appears. Actual results: Nothing happens. Notes/Issues: Verified on FF37 (Win 7). Issue is reproducing on MP-stage and MP-dev. Screencast for this issue http://screencast.com/t/MhSEzhgIPa

Also, when user clicks “Attach another file” after he already posted a first note, in “My Threads” Comm Dash page (https://marketplace.allizom.org/comm), more than one line containing “Attachment” and “File Description” fields, appear (http://screencast.com/t/MhbGzXzv3qKo). Is this the same issue?
