This bug is the one I'm least happy about from a UX perspective. It just seems like we're awkwardly fitting this into our flow. So, apologies ahead of time and I'm open to suggestions. That said:
Our best idea was to create a section at the top of the Status & Versions page, similar to what we do with the Marketplace. See attachment for a mockup.
* There is no submit button because each radio button (and the delete add-on button) actually have a javascript pop-up to confirm the change. (This sounds like something that won't be accessible - I'm not actually sure what screenreaders do with JS popups...)
* We're changing the name of "Disable" to "Hidden." So, hidden isn't a new option, it just means we're disabling the add-on.
* See the links in the left menu for disabling and deleting add-ons? If you click on those today there are two pop-ups. We want to use those same pop-ups on the new Hidden and Delete links.
* Now that Hidden and Delete are in the middle of the page, we should remove the old links from the side bar.
* The Published and Unlisted options should have a popup with "Confirm" and "Cancel" buttons and text that says: (will ni?jorgev to fill this in)
The actual actions of switching to published or unlisted are covered in bug 1122107 and bug 1122108
Edit: Actually, can we call this Listed instead of Published? I already made the screenshot but I think that would be more consistent.

Listed/Unlisted makes sense to me. We used to have "listed" add-ons which weren't hosted (fun!), but we discontinued those long, long ago.

Update after our meeting today:
1) This will be a read-only option, no <form> and no changes on the page
2) The page it's on and location is fine
3) The UI can change (doesn't need to be radio buttons), it just needs to say what the status is.
