Steps to reproduce: 1. Load MP-dev Statistics page https://marketplace-dev.allizom.org/statistics/ 2. Go to page footer and click the “Feedback” link Expected results: The feedback page is loaded containing a text filed ,“Cancel” and “Submit feedback” buttons. Actual results: There is no cancel option, and the “Submit feedback” button is too large. Notes/Issues: Verified on FF38 (Win 7). Issue is reproducing on MP-dev and MP-stage. Also, let's revise the text: "let us know if whether" is no bueno.

p4 because it doesn't look too dreadful and is only on desktop when going from the statistics page.
