Presumably, when a user's iOS preferences select a language, iOS automatically delivers both app UI and web content in that language. From this screenshot, you can see that iOS has respected my language preferences (Italian) and given me the Italian app UI localization in the iOS simulator, but when navigating to mozilla.org, I'm fed en-US content. This content should be in Italian, not English.

Does this work on-device?
I recall some oddities in the Simulator's handling of locales, so trying to decide if this is a bug-bug or a doc-bug.
OS: Mac OS X → iOS 8
Hardware: x86 → All
Summary: accept-language preferences are not being respected while browsing in iOS simulator → Accept-Language preferences are not being respected while browsing in iOS simulator

None of us (l10n-drivers) are part of the TestPilot program yet (I don't have an iOS device), so we can't confirm if this is a bug-bug.

I ended up installing XCode and trying, seems to work fine here.
One thing I'm noticing: you should at least see "Gestore", not "Carrier" in the top left corner, so something might not be right in your simulator.

Just tried it on device and accept-lang header settings are respected properly.
