Why do you expect the bookmark to be removed? Ctrl+D brings up the “Page Bookmarked” pop-up panel, for which Alt+R doesn't apply to anything. The “Remove Bookmark” button is only available in the “Edit This Bookmark” pop-up panel.
Did you actually mean this?
1. Ctrl+D.
2. Enter.
3. Ctrl+D.
4. Alt+R.

Yep, that's what I meant. They look similar enough that I mistook the second for the first. Sorry about that.

Okay, redoing the description then:

Unity 7.3.2
Steps to reproduce:
1. Press Ctrl+D. The “Page Bookmarked” panel pops up.
2. Press Enter to save the bookmark.
3. Press Ctrl+D. The “Edit This Bookmark” panel pops up.
4. Press Alt+R to activate the “Remove Bookmark” button.
Expected results:
The bookmark should first be added, then removed.
Actual results:
The bookmark is first added. The bookmark is not removed; instead, the “Edit This Bookmark” panel simply closes and focus goes to the menubar.
