Had this problem in https://bugzilla.mozilla.org/show_bug.cgi?id=978846#c23 and below. I tried logging out then back in, removing the reviewer and re-adding, the problem persisted.
You were reviewing a child commit; at the moment, you have to resolve the parent for an r+. We fully recognize this is confusing. This behaviour will be changing within a week or two; see bug 1055021.
